As a Certified Family Nurse Practitioner with years of experience in family medicine, urgent care, and bedside nursing, I am passionate about making healthcare more accessible, convenient, and patient-centered. My goal is to build trusting relationships with my patients while delivering personalized care that prioritizes your health, comfort, and well-being. Virtual Sick Visit

$65

Quickly assess common illnesses such as cold, flu, sinus infections, sore throats, rashes, and urinary tract infections. Includes a full evaluation, medical recommendations, and prescriptions (if appropriate) sent directly to the pharmacy of your choice. 

Chronic Condition Management-Initial Visit

$100

Designed to help gain control of ongoing conditions such as hypertension, diabetes, GERD, thyroid imbalances, if hyperlipidemia. Includes a look into your medical history, lab results, lifestyle, and medications. 

Chronic Condition Management-Subsequent Visit

$85

Tracks your progress, review lab results, and make adjustments to keep health on track. Whether you're managing diabetes, thyroid issues, or high blood pressure, the nurse practitioner will address any concerns and make any adjustments needed to be made.

Women's Health Visit

$75

Comprehensive support for unique needs such as, yeast infections, sexually transmitted infections, urinary tract infections, bacteria vaginosis, overactive bladder, and birth control consultation or refills. This visit focuses on open communication and personalized care. The nurse practitioner will review your history, address concerns, and create an individualized plan to support your overall health. 

Men's Health Visit

$75

Comprehensive support for unique needs such as erectile dysfunction, low testosterone, BPH, overactive bladder, and premature ejactulation. This visit focuses on open communication and personalized. The nurse practitioner will review your history, address concerns, and create an individualized plan to support your overall health. 

Mental Health Management

$100

Virtual mental health evaluations to support anxiety, depression, stress, or insomnia from the comfort and privacy of your home. The nurse practitioner will help identify any underlying causes and develop an individualized plan to balance emotional needs. 

Weight Loss and Wellness

$150

This service provides a complete baseline assessment of your metabolic health, including baseline labs and a 1 on 1 consultation with the nurse practitioner. During this consultation, the nurse practitioner will review lab results and identify factors that plays a role in your weight. Together, we will come up with an individualized weight loss plan designed just for you!

Monthly GLP-1 Program (Tirazepatide/Semaglutide)

$100 (+$20 each dose increase)

This program offers medically supervised treatments for qualified patients seeking effective, long-lasting results. Tirzepatide and Semaglutide are both FDA approved medications used in this program. This program includes consultation, prescription, and supplies-combined with lifestyle changes. 

Medical Documentation

$95

  • Workplace Accommodation Letter: ADA compliant documentation including evaluation of condition, discussion of workplace challenges, and official letter to your employer. 
  • Handicap/Disabled Parking Permit: If you have a medical condition that limits your ability to walk or move safely, I can provide documentation to qualify you for a disability parking placard. 
  • Emotional Support Animal Letter: If you struggle with anxiety, depression, or PTSD, a letter will be provided to support documentation. 


School and work excuse notes may be provided when medically appropriate following a completed telehealth visit. Documentation will reflect the dates you were evaluated and any recommended time needed for recovery based on the provider’s clinical judgment. Excuses cannot be issued without an evaluation.
